RN Hospice Liaison & Care Coordination

Hospice of Orange & Sullivan Counties, Inc. seeks an experienced Registered Nurse Clinical Liaison to join our Patient Access team. This role combines clinical expertise with relationship-building, education, and care coordination to expand access to hospice services throughout our community. As an RN Clinical Liaison, you will educate referral sources, conduct hospice evaluations, support admissions, and collaborate with an interdisciplinary team to ensure timely, compassionate transitions to #J-18808-Ljbffr Hospice of Orange and Sullivan Counties, Inc.
