Description

• Conduct patient evaluations in both inpatient and outpatient environments and create tailored treatment strategies
• Implement physical therapy interventions concentrating on mobility, strength training, balance, and overall function
• Work collaboratively alongside nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge planning, and at-home exercises
• Maintain thorough documentation of assessments, treatments, and patient progress in line with facility requirements
• Engage in care team meetings and interdisciplinary discussions to ensure seamless care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (such as neurological or geriatric) are welcomed but not mandatory

Skills

• Prior experience is strongly preferred
