PHYSICAL THERAPIST HFM, NEMS - SHEBOYGAN NORTH REHAB

Position Details Location: US:WI:SHEBOYGAN at our NE-SHEBOYGAN NORTH TAYLOR CLINIC facility. This is an on‑site position. Full‑time equivalent: 1. Standard hours: 40. Shift: Shift 1. Shift details: Anywhere from 7 am – 5 pm; flexible and willing to work with candidate. Holidays: None. Weekends: None. Job Summary Evaluates and treats all referred Physical Therapy patients according to physician orders. Home care: Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients. Qualifications Education : Degree in Physical Therapy required. Licensure : Wisconsin licensed Physical Therapist or eligible required. If eligible, must obtain Wisconsin license within 3 months of hire. CPR certification is required. Experience : At least one year experience in a medical setting preferred. Special Skills : Good communication skills required.
