Primary Care Physician

Join a physician-led primary care organization built around outcomes and relationship-driven care as a Primary Care Physician (MD/DO). This role offers manageable patient panels, comprehensive interdisciplinary care team support, and the ability to practice medicine with time, accountability, and purpose. Clinicians will provide comprehensive primary care to older adult patients within a value-based Medicare Advantage model while focusing on prevention and long-term health improvement.
