RN Case Manager: Discharge & Care Coordination

Ochsner Health in Louisiana seeks a Registered Nurse to support discharge planning, utilization review, and care coordination during acute hospitalizations. You will review hospital stays, collaborate with ED physicians, hospitalists and care teams, and help develop discharge plans that meet patient needs while ensuring compliance with CMS guidelines and documentation standards.
