Hospice RN Case Manager: Lead Compassionate Care

Gentiva Heartland Hospice in Oklahoma seeks a dedicated Hospice RN / RN Case Manager to deliver compassionate, patient-centered care in patients' homes or other settings. You will guide families, document care, and collaborate with physicians, LPNs, CNAs, social workers and chaplains to implement personalized plans of care.
