Advanced Practice Provider - Primary Care - Mount Prospect

Position Details Hourly Pay Range: $56.44 - $70.58. Position: Advanced Practice Provider (APN/PA) – Primary Care (Family Medicine). Employer: Endeavor Health. Location: 199 W. Rand Rd, Mount Prospect, IL. Full Time, 40 hour week. Hours: Monday - Friday, 7:00am - 5:00pm. Rotating Saturday's, 8:00am - 12:00pm. Rotating Call – Approx. one week/year. Responsibilities Under the direction of collaborating physician(s), provide care to patients. Obtain detailed history and perform physical examinations; order imaging, laboratory tests, and diagnostic procedures, and interpret results. Instruct patients on prescribed therapeutic regimens and pre/post operative instructions. Maintain records and write progress notes on status, treatments, and procedures. Follow patients through their course of treatment in clinic and hospital settings. Collaborate with physicians and the health care team to achieve optimal patient outcomes. Prescribe pharmacologic and non-pharmacologic therapies as appropriate. Demonstrate evidence‑based practice. Participate in the development, implementation, and evaluation of standards of practice in the specialty. Assess the learning needs of patients and families across the continuum. Employ strategies, interventions, and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnoses, and prescribe treatments under protocol in the direct management of acute and chronic illness and disease. Promote wellness and prevent illness and injury in various settings for individuals, families, and communities. Work autonomously and in interdisciplinary teams as resources and consultants. Positively contribute to safety outcomes and promote high‑quality patient experience. Participate in the development, implementation, and evaluation of service line goals. Support the strategic plan of the organization. Adhere to all Endeavor Health policies and procedures. Qualifications License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required. Completion of master’s level program in specialty area. BLS/CPR certification required. Minimum 2 years APN/PA experience in Primary Care setting required. Preferred: experience in acute, chronic disease, and preventative care. Excellent communication skills and empathy/compassionate patient care approach.
