The Advance Practice Provider is responsible for the home visits, TCM visits, medication optimization, standardized assessments, patient self-scheduling, and care team coordination. With our annual performance bonus, you can earn up to 15% of your base salary.Full Benefits OfferedAnnual Performance Bonus based on Quality of CareNegotiable Relocation & Sign-On BonusesMalpractice Insurance CoverageFull support both in-clinic and through our Central Business Office to ensure providers can focus solely on patient careResponsibilitiesCan work part-time in the clinic and part-time seeing patients at home.Complete and submit all encounter plans within 72 hours of each visit endingPerform comprehensive medication management using the R.O.C. framework (Reconcile, Optimize, Comply) at every visitAdminister all required standardized clinical screenings: PHQ-2/9, Mini-Cog, STEADII, REALM-SF, and Zarit Caregiver InterviewEscalate clinical needs to VMH Physician and/or Social Worker via patient case forward in EClinicalWorksSelf-schedule the next visit before leaving each appointment; write next visit date on the Patient Instruction Sheet and place it on the patient's refrigeratorConduct telephonic outreach between visits as clinically indicatedPerform acute care visits as clinically warranted to prevent unnecessary ED utilizationQualificationsCertification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC)Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) program, focusing on adult medicineCertification through an accredited PA certification program focusing on adult medicineCurrently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employmentIdeal Candidate, with a minimum of two years of experience as Nurse PractitionerBilingual English Spanish speaking preferredHolds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)Up-to-date CPR Certification and Basic Cardiac Life Support certificationRequired SkillsStrong organizational and prioritization skillsAbility to use Electronic Medical Record (EMR) system2 years of experience of Medical Advantage, HEIDS, MRA and ICD-10 CodingComfortable managing a primarily adult-aged patient populationDemonstrated ability to problem-solve complex and multifaceted situationsPreferred SkillsBilingual (English-Spanish) PreferredPay range and compensation packageCompetitive Salary + Annual Quality Bonus – Our base salary range for this position is $125,000 - $135,000, based on experience.
