Description

American Traveler is seeking an experienced Home Health RN for a 13-week travel contract in Plymouth County, Iowa, requiring at least 3 years of experience with OASIS assessments and home-based care planning.

Details

• Home health setting serving patients in their homes across Plymouth County, Iowa
• Patient care involves OASIS assessments and individualized care planning for in-home patients
• Scheduled shifts are days, Monday through Friday
• One in four call rotation covering nights, weekends, and holidays
• 32–40 hours per week
• Epic EMR experience is preferred

Requirements

• Active Iowa RN license or Compact RN license required
• Minimum 3 years of RN experience required
• At least 2 years of experience performing OASIS assessments and care planning for in-home patients required

Additional Information

• Responsible for the comprehensive nursing care of patients in their home environment
• Participates in the department's on-call rotation for nights, weekends, and holidays (one in four)
