Registered Nurse (Referral Coordination Team)

Duties of the Registered Nurse (Referral Coordination Team) include but are not limited to:

• Communicate with Veterans regarding their health care options including local VA Medical Center and community-based eligibility, allowing the Veteran to make the most informed decision for Right Care, Right Time, at Right Location.
• Serve as a liaison between referring providers and specialty care services.
• Outpatient specialty clinic care coordination and patient flow management, including appointment management.
• Facilitate the timely and clinically appropriate processing of referrals throughout the facility and region.
• Clinically review internal consults/referrals for medical necessity of requested services using clinical judgment, triage tools, and care coordination agreements.
• Collaborate closely with clinicians and leaders from the medical and surgical specialties, primary care, and Community Care, to facilitate timely and appropriate care.
• Provide clinical knowledge and act as a resource to multidisciplinary staff.
• Track consults previously cancelled due to incomplete pre-work and ensure required pre-work is completed, and consults are resubmitted as appropriate.
