Clinical Pharmacist 2 (Nephrology)

The Clinical Pharmacist 2 (Nephrology) is an integral part of the outpatient dialysis care team, providing advanced pharmacy services and managing drug therapy under established protocols. The pharmacist collaborates closely with nephrologists, nurses, and dietitians to improve medication-related outcomes for patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), kidney transplants, and other renal conditions. This role acts as a valuable resource to support healthcare professionals, including pharmacists, pharmacy students, and residents.

This position is on-sight and located at the Baylor Scott and White Dialysis Center in Temple, TX.

Key Responsibilities:

• Direct patient care: Provide comprehensive medication management, including medication reconciliation along with initiating and adjusting drug therapies in collaboration with physicians.
• Disease state management: Manage and optimize pharmacotherapy for kidney disease and related conditions such as:
• Hypertension
• Diabetes
• Anemia of CKD
• Mineral and bone disorders
• Hyperkalemia
• Anticoagulation
• Chronic pain and palliative care.
• Patient education: Provide counseling to patients and their caregivers on medication use, side effects, dietary restrictions, lifestyle modifications, and self-management techniques.
• Transitions of care: Assist with managing medication therapies for patients moving between different care settings, such as hospital discharge, outpatient clinic visits, and dialysis.
• Interdisciplinary collaboration: Work as a key member of the outpatient dialysis care team, providing expert consultation and participating in patient rounds and case reviews.
• Quality improvement: Conduct Medication Use Evaluations (MUEs), monitor for and report Adverse Drug Reactions (ADRs), and participate in continuous quality improvement initiatives.
• Staff and resident education: Serve as a preceptor for pharmacy students and PGY1/PGY2 pharmacy residents, providing training and mentorship.
• Clinical documentation: Maintain thorough and accurate documentation of all patient care activities in the electronic health record (EHR).
• Process improvement: Coordinates and develops drug therapy management protocols and workflows related to the care of patients with ESRD and dialysis.
• Medication access: Support prescription prior authorizations as needed
