Occupational Therapist

Job TitleAssess and develop interventions for mitigation of fall risks and adaptive equipment including but not limited to the following:Assess functional mobility to determine needs for mobility equipmentNotify FSH of equipment neededProvide consumer/caregiver education on appropriate use and safety with adaptive equipmentOther interventions to mitigate fall risks as recommendedProvide additional OT assessments and interventions including but not limited to those related to:Activities of daily living/self-careFine motor coordinationGeneralized weaknessMuscle re-educationPain managementUp to 24 hours per week.
