HealthCare Support is actively seeking a to Nurse Practitioner / Physician Assistant fill an opening in Windsor, Connecticut. Daily Responsibilities for Nurse Practitioner / Physician Assistant:
- Conduct comprehensive assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions
- Communicate and collaborate with the interdisciplinary care team
- Monday to Friday 8AM - 5PM
- Active and unrestricted license in New York
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
