Civil Service Permanent Full-Time Department of Human Services Union SEIU Local 668 Bargaining Unit Bureau / Division Code Bureau / Division Norristown State Hospital Talent Management Division As a Social Worker 1 with the Department of Human Services (DHS) at the Norristown State Hospital, you become part of a team that stands beside patients as they work toward stronger and more independent lives. You assist with case planning, help patients build stability, and guide thoughtful transitions into community settings. This role supports patients receiving court-ordered mental health treatment in the Civil/Step-Down Units by coordinating care, planning treatment needs, and facilitating communication with families, legal representatives, and community agencies. You work as part of a multidisciplinary team to help patients prepare for successful transitions and continued progress. Conduct psychosocial assessments, organize findings in the Electronic Health Record ( EHR), and complete related forms Transport patients to interviews, community placements, and follow-up appointments Documentation Compliance: Maintain current records, meet regulatory standards, and ensure accuracy in treatment documentation Utilize community resources, coordinate with outside providers to ensure continuity of care, and identify new resources as appropriate Full-time employment, 37.5 hours per week Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Overtime and travel as needed Telework: You will not have the option to telework in this position. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY Minimum Experience and Training Requirements: A bachelor's or master's degree in social work or social welfare.
