At Lifted Hospice, our Clinical Navigators help patients, families, and providers take the first step into hospice carewith clarity, compassion, and confidence.
As a Clinical Navigator, you'll combine your nursing expertise with strong communication and outreach skills to assess eligibility, provide education, coordinate admissions, and facilitate seamless transitions into hospice care. You are often the first clinical face patients seebringing reassurance, presence, and peace to what can be an overwhelming moment.
You'll also serve as a trusted clinical liaison between patients, families, referral partners, and the interdisciplinary team, ensuring communication remains clear throughout the admission process. In addition, you'll play an essential role in facilitating and managing General Inpatient (GIP) admissions, helping patients receive the appropriate level of care while maintaining the highest standards of clinical excellence and regulatory compliance.

Your Role in the Mission
As a Clinical Navigator, you combine clinical expertise with compassionate guidance to help patients and families navigate the transition into hospice care. You assess eligibility, provide education, coordinate admissions, and collaborate with physicians, the Medical Director, Hospice Liaisons, and the interdisciplinary team to ensure timely, seamless care.
You also play a key role in General Inpatient (GIP) care by evaluating eligibility, coordinating admissions, conducting daily GIP visits, and maintaining accurate, compliant documentation. Through education, relationship-building, and clinical excellence, you strengthen referral partnerships while representing Lifted Hospice with compassion, integrity, excellence, innovation, and joy.
Key Responsibilities
Patient and Family Navigation
- Provide hospice education Guide patients, families, and caregivers through hospice services, levels of care, and what to expect while offering compassionate support throughout the admission process.
- Conduct clinical assessments Evaluate patients for hospice eligibility and facilitate timely, seamless admissions.
Clinical Assessment & Admissions
- Coordinate admissions Collaborate with physicians, the Medical Director, and the interdisciplinary team to ensure patients transition smoothly into hospice care.
- Complete accurate documentation Maintain timely, compliant documentation that supports hospice eligibility and regulatory requirements.
General Inpatient (GIP) Care
- Support GIP services Assess patients for General Inpatient (GIP) eligibility, coordinate admissions, conduct daily GIP visits, and ensure documentation supports the appropriate level of care.
Referral Partner & Community Relationships
- Build trusted partnerships Develop relationships with hospitals, physicians, case managers, discharge planners, and other referral partners while providing education about Lifted Hospice services.
- Support outreach efforts Partner with Hospice Liaisons to strengthen referral relationships through clinical education and community engagement.
Quality & Compliance
- Ensure quality and compliance Uphold Medicare, CHAP, HIPAA, and Texas regulatory standards while maintaining accurate EMR documentation and delivering compassionate, patient-centered care.
Skills & Qualifications
- Current, unrestricted RN license in the State of Texas, in accordance with Nurse Licensure Compact rules.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 2 years of clinical nursing experience required.
- Experience with hospice admissions and General Inpatient (GIP) level-of-care management preferred
- Experience collaborating with physicians, hospitals, and interdisciplinary healthcare teams.
- Strong understanding of hospice philosophy, eligibility criteria, Medicare regulations, and Conditions of Participation (CoPs).
- Working knowledge of CHAP standards and Texas hospice regulations.
- Knowledge of General Inpatient (GIP) documentation requirements and level-of-care criteria.
- Excellent clinical assessment, critical thinking, and decision-making skills.
- Ability to navigate emotionally sensitive conversations with empathy, professionalism, and confidence.
- Strong communication, organizational, and time-management skills.
- Proficiency with electronic medical records (EMR) and Microsoft Office applications.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Must be able to read, write, and communicate clearly in English.
- Current driver's license, reliable transportation, and valid auto liability insurance.
Where You'll Work
As a Clinical Navigator, you'll spend most of your time in the fieldmeeting patients and families at the bedside, supporting hospice admissions, conducting clinical assessments, and facilitating General Inpatient (GIP) care when needed. You'll visit hospitals, skilled nursing facilities, assisted living communities, and private homes, providing clarity, compassion, and clinical expertise during the beginning of the hospice journey.
You'll also collaborate with Hospice Liaisons and your local team of nurses, social workers, chaplains, and physicians to strengthen outreach, build trust, and align clinical messaging across your territory. This role requires flexibility, adaptability, and professionalism in diverse environments and schedules.
