(RN) Care Manager - Per Diem - Care Management - 140292

We are a tertiary level acute hospital where Care Managers coordinate care for medically complex patients, utilizing critical thinking and discharge planning skills. The Care Management Department includes highly skilled Care Managers and Social Workers dedicated to providing excellent care for our diverse patient population. Under general supervision, the RN Care Manager facilitates the patient’s hospitalization from pre‑admission through discharge. The Care Manager coordinates with physicians, nurses, social workers and other healthcare providers to expedite medically appropriate, cost‑effective care. They apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning and communicate with administration and the healthcare team as needed. Minimum Qualifications Graduate from an accredited school of Nursing; ADN with at least 3 years of acute care manager experience will be considered. Registered Nurse (RN) license issued by the State of California. BLS at time of hire; commitment to obtain BART within six (6) months. Minimum of 3 years prior inpatient RN care manager experience in an acute care hospital, or equivalent inpatient RN experience, within the last 7 years. Demonstrated utilization review/discharge planning experience. Effective interpersonal communication and negotiation skills, leadership ability, and conflict management and resolution. Thrives in a fast‑paced, dynamic environment and is committed to professional growth and ongoing departmental performance improvement. Demonstrated understanding of patient flow, transitions to post‑acute care and community‑based services. Proficiency in information systems, databases, and programs including MS Office: Word, Excel, Outlook, and PowerPoint. Fast and accurate typing, data entry skills, and proficiency with detailed written communication appropriate for electronic medical records. Preferred Qualifications

BSN/MSN

ACM and/or CCM certificate. Three to five (3‑5) years clinical RN experience or RN care management experience in an acute care setting. Med/Surg, progressive care, or ED clinical RN experience. UCSD RN experience or RN experience in another academic medical center. Previous supervisory/management experience. RN experience in a post‑acute care facility or home health agency. Facile with Epic, ECIN (Allscripts) and/or Utilization Review / InterQual. Knowledge of local payers and coverage, reimbursement, and evidence‑based clinical nursing practice. Special Conditions Must be able to work various hours and locations based on business needs. Employment is subject to a criminal background check and a pre‑employment physical exam. Pay Details Annual Full Pay Range: $181,990 – $181,990 (will be prorated if the appointment percentage is less than 100%). Hourly Equivalent: $87.16 – $87.16. Compliance If employed by the University of California, you will be required to comply with our Policy on Vaccination Programs, which may be amended or revised from time to time. Federal, state, or local public health directives may impose additional requirements. Life‑support certifications (BLS, NRP, ACLS, etc.) must include hands‑on practice and in‑person skills assessment; online‑only certification is not acceptable.
