Provide comprehensive mental health assessments, treatment and assertive outreach support Develop and implement individualised care plans in partnership with consumers and carers Build strong relationships with families, NGOs and healthcare providers Advocate for consumers and facilitate access to services and community resources Participate in clinical reviews, quality improvement initiatives and service development within our new 7-day model Help shape the future of community mental health care. Essential Requirements: Qualification: Relevant tertiary qualifications as a Registered Nurse Registration: Demonstrated full registration with AHPRA as a Registered Nurse Selection Criteria: Demonstrated skills in mental health and discipline specific assessments and interventions and experience working with people with severe and persistent mental illness. Demonstrated understanding and ability to work within experience working in a team and/or knowledge of the principles that guide service delivery within this model of practice Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in mental health service provision. Demonstrated working knowledge of NSW Mental Health Act, 2007. Demonstrated excellent communication skills including verbal, written, and good interpersonal skills.
