Home Health Aide Job Opportunity

At Modani Care, we are dedicated to providing Home Care Services You Can Trust to seniors and adults who need support with daily living. Our team helps families understand care options, navigate state-specific programs, and receive personalized support that allows individuals to remain safe and comfortably at home. We are seeking a compassionate and experienced Home Health Aid to join our team in Centralia, MO .

Responsibilities

Provide personal care and daily living assistance to our clients, including bathing, dressing, grooming, and toileting
Assist with activities of daily living, such as meal preparation, medication management, and mobility support
Monitor and report any changes in client condition or needs to the supervising nurse and/or physician
Maintain a clean and safe living environment for clients
Collaborate with other healthcare professionals to ensure comprehensive care
Document all care activities and client interactions accurately and thoroughly

Requirements

Minimum of 2 years of experience as a Home Health Aid or related healthcare field
Ability to lift 25 pounds and assist with transfers and ambulation
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation
Compassion and empathy for clients and their families
