Physical Therapist PRN

Role: Physical Therapist responsible for assessing, planning, and implementing rehabilitative programs to improve patient mobility, reduce pain, and prevent deformities. Services are delivered across the continuum with a focus on quality, cost-effectiveness, and regulatory compliance. Key Responsibilities Patient Assessment: Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge strategies, involving patients and families. Treatment Planning & Care: Implement therapy plans, select appropriate activities, revise goals as needed, and provide patient/family education and discharge instructions. Documentation: Record evaluations, treatment sessions, and discharge details following departmental protocols. Professional Conduct: Maintain ethical standards, confidentiality, punctuality, dress code adherence, effective communication, and participation in meetings and training. Customer Service & Professional Development: Deliver excellent service, foster teamwork, participate in ongoing education, and maintain licensure and certifications. Additional Duties: Perform assigned tasks and comply with organizational policies. Qualifications & Skills Masters or Doctorate in Physical Therapy (preferred). Licensed as a Physical Therapist and BLS certified. Minimum 1 year of experience in rehab, LTACH, acute care, or SNF settings.
