Case Manager-(HRA) SS-Bronx

Comunilife is looking for experienced Case Managers to fill open positions for their HRA Scatter Program in the Bronx. The Case Manager’s main responsibility is to provide case management services for a caseload of approximately 22 clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required.
