Advanced Practice Provider

PremierCardia is a physician-led cardiology service provider dedicated to delivering specialized cardiovascular care to post-acute care facilities. Serving long-term acute care hospitals, skilled nursing facilities, assisted living communities, and rehabilitation centers, we address the unique challenges of these environments, including limited access to specialized services and reducing hospital readmission rates. Our patient-centered care model emphasizes evidence-based treatments, innovative diagnostics, and compassionate support. We provide in-person support and partnership through our cardiologists and advanced practice providers, while managing billing and testing authorization services at no cost to partner facilities. Role Description This is a full-time, on-site role for a Post Acute Cardiac Advanced Practice Provider (APP) located in Jacksonville, FL and surrounding area. The APP will collaborate with physicians and healthcare teams to provide patient-centered cardiac care. Day-to-day responsibilities include conducting patient assessments, diagnosing and managing cardiovascular conditions, creating and implementing care plans, performing follow-up evaluations, and maintaining accurate medical records. The APP will also educate patients and staff on cardiac care practices and ensure adherence to evidence-based clinical guidelines in a post-acute care setting. Qualifications Proficiency in patient evaluations, diagnosis, and management of cardiovascular conditions Experience in developing and implementing personalized care plans and assisting with post-acute care needs Knowledge of evidence-based medical practices, diagnostics, and treatment protocols in cardiology Strong communication and patient education skills to foster collaborative care environments Active Nurse Practitioner (NP) license in the state of Florida Cardiology specialization or previous experience in post-acute care settings preferred Ability to work effectively in an interdisciplinary healthcare team Current ACLS/BLS certifications required #J-18808-Ljbffr PremierCardia
