Job Opportunity: Licensed Practical Nurse

Our organization is looking for a dedicated and empathetic Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation facility located in Dayville, CT. This role is essential in delivering exceptional clinical care to our residents.

Key Responsibilities

Provide direct patient care to residents.
Administer medications and perform necessary treatments.
Monitor and assess the health conditions of residents.
Maintain accurate and thorough documentation in medical records.
Collaborate with an interdisciplinary team to support individualized care plans.
Communicate effectively with residents and their families.
Ensure adherence to standards of dignity, safety, and infection control.

Qualifications and Requirements

Valid LPN license.
Compassionate approach to patient care.
Ability to work collaboratively within a team.
Strong communication skills.
