RN Hospice Full-time

About This Role As the Registered Nurse Case Manager for Christus VNA Hospice, you will take full nursing responsibility for the delivery of the Plan of Care for each hospice patient, collaborating with other professionals to ensure comprehensive care. Highlights Location: San Marcos, TX Schedule: Full Time Pay: $83,580 - $125,370 annually What You'll Do Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral. Identify the patient's/family's physical, psychosocial, and environmental needs, reassessing no less than every 15 days. Initiate and coordinate the plan of care. Document problems, appropriate goals, interventions, and patient/family responses to hospice care. Collaborate with the patient/family, attending physician, and other members of the IDG in providing care daily. Instruct and supervise the patient/family in self-care techniques when appropriate. Maintain accurate and relevant clinical notes regarding the patient's condition. Requirements Education Current and unrestricted RN licensure in the state of practice. Experience 1+ years of clinical experience preferred. Licenses & Certifications Current driver's license and access to a dependable vehicle or public transportation.
