In this vital role, you'll coordinate patient services, support our interdisciplinary care team, and help deliver the highest quality end-of-life care. The hourly range for this position at commencement of employment is expected to be between $32 - $36; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience. What You'll Do

• Coordinate daily hospice patient schedules and services
• Manage patient referrals, admissions, and intake processes
• Obtain physician orders and verify insurance benefits/authorizations
• Support interdisciplinary team meetings and coordinate patient care plans
• Monitor documentation for quality, accuracy, and regulatory compliance
• Prepare and submit required hospice quality and HIS data
• Serve as a resource for clinical staff regarding hospice patient care
• Collaborate with case managers to ensure continuity of care
• Assist with staff orientation, education, and performance improvement initiatives
• Maintain patient confidentiality and uphold hospice standards of excellence

Qualifications

• Current California LVN or RN license preferred
• Minimum 2 years of hospice or home health patient coordination experience
• Knowledge of California hospice regulations and Medicare/Medicaid guidelines
• Current CPR certification
• Strong communication, organization, and multitasking skills
• Proficient with computers and electronic medical records
