RN/LVN Clinical Coordinator/Boerne

LVN Clinical Coordinator – Boerne, TX Oversees daily clinical operations, manages patient schedules, and ensures care aligns with regulatory standards. Acts as a liaison between nurses and therapists, handling physician orders and conducting chart audits. Interacts with administrative and clinical staff, including front desk, medical records, nursing, and ancillary services when applicable, to maximize resource utilization and ensure efficient delivery of services. Coordination & Compliance Ensures clinical services are delivered per the patient’s care plan and meet state and federal regulations. Reviews clinical notes. Communication Manages referrals, takes verbal or written orders from physicians, and updates At Risk Registries. Software Proficiency Utilizes industry‑standard EMR systems such as Kinnser/WellSky. Qualifications Education: LVN Certified Experience: Minimum 1–2 years of home‑health clinical experience, including wound care. Skills & Abilities Strong leadership and effective communication skills. Excellent planning, organization, and management abilities. Ability to multi‑task and demonstrate teamwork cooperation. Manages medical supplies orders, distribution, and inventory. Employment Details Full‑time, Monday through Friday with a rotating on‑call schedule assigned.
