Ambulatory Pharmacist II

Cambridge Health Alliance seeks an Ambulatory Pharmacist II to optimize medication therapy for diverse, underserved patients in our outpatient clinics. In this academic, team-based setting, you’ll provide comprehensive medication management, chronic disease state management (e.g., diabetes, hypertension, anticoagulation), and patient/caregiver education. Collaborate closely with physicians, nurses, and behavioral health providers to develop evidence-based care plans, perform medication reconciliation and safety reviews, support population health and quality initiatives, precept learners, and advance health equity in a mission-driven, teaching-focused environment.
