In this role, you will: - Assess patients in either an inpatient or outpatient setting and craft personalized treatment plans tailored to their needs. - Deliver physical therapy interventions that aim to enhance mobility, strength, balance, and overall function. - Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes. - Educate patients and their families about therapeutic goals, discharge planning, and home exercise recommendations. - Maintain thorough documentation of evaluations, treatments, and patient progress in compliance with both facility and regulatory standards. - Actively engage in care team discussions and interdisciplinary conferences to ensure cohesive and coordinated patient care. To qualify for this rewarding role, you will need: - A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program. - Current state licensure as a Physical Therapist. - CPR certification is preferred. - Specialized certifications (e.g., in neurology or geriatrics) are valuable, but not mandatory.
