Physical Therapist – Outpatient Rehabilitation

Schedule: Days 8 hours, 40 per week

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Valid physical therapy license
• Certifications as required by state regulations
• Experience in administering physical therapy treatments

Roles & Responsibilities:

• Administers various standard physical therapy treatments to alleviate pain, restore motor function, and enhance performance.
• Completes patient evaluations and discharge forms within specified timelines.
• Documents patient assessments and care in accordance with agency policy and state regulations.
• Coordinates with healthcare team members and prepares patients for discharge.
• Supervises treatment administration by Physical Therapy Assistants and aides.
• Participates in case conferences and staff meetings as required.

Compliance / Notes:

• Maintains compliance with departmental policies including attendance, confidentiality, and continuing education.
• Participates in the Performance Improvement Plan activities as requested.
• Responsible for training and orienting new staff as necessary.
