Join LCMC Health, and youll find that our everyday makes it easy to live your extraordinary./ppPosition Summary/ppThe RN Transfer Nurse is responsible for coordination and communication of transfer requests for LCMC Health Adult Hospital Market (AHM) facilities by using problem-solving, critical thinking, and excellent customer service skills. This professional will act as a facilitator and decision-maker while applying nursing knowledge as a foundation for placement decisions and communications. This role will also be responsible for bed planning and throughput facilitation at designated LCMC Health Adult Hospitals./ppGeneral Duties/pulliServe as the point person to facilitate incoming transfers/transports, maintain communication with all parties including but not limited to attending/accepting MD and referring facility./liliUses problem-solving skills to determine best course of action which will be satisfactory to the patient and referral health care team and optimize communication with the appropriate physician./liliUtilizes nursing experience and education as foundations for gathering accurate clinical data to assure appropriate and prompt transfer and placement of the patient./liliResponsible for documenting, tracking, and coordinating logistics for hospital transfers and bed planning./liliEstablishes and maintains a high degree of awareness and knowledge of hospitals medical staff, services and programs./liliMaintains professional and technical knowledge through continuous education and accepts new assignments and performs additional duties as requested./li/ulpExperience Qualifications/pulli3 years experience in inpatient nursing / call center / critical care / ED/li/ulpLicenses and Certifications/pulliAmerican Heart Association Basic Life Support/CPR/liliLicensed Registered Nurse through the Louisiana State Board of Nursing/li/ulpSkills and Abilities/pulliBasic Nursing skills with high level critical thinking ability./liliExhibits excellent customer relationship skills including telephone and personal communication with all customer groups (physicians, administrators, nurses, case managers, and other team members)./liliProficient in hospital computer systems for accurate data collection, monitoring and evaluation./li/ulpReporting Relationships/pulliDoes this position formally supervise employees?
