The Case Management Manager leads and supports theCase Management team in coordinating patient care from admission throughdischarge. This role collaborates closely with physicians, nursing staff, and interdisciplinaryteams to remove barriers to timely discharge, improve patient outcomes, ensureregulatory compliance, and promote cost-effective care while maintaining thehighest standards of patient advocacy What You’ll Do Key Responsibilities Conductdischarge assessments on admission & develop timely, appropriatedischarge plans Performdaily patient assessments, monitor, evaluate & document patientprogress Coordinatepost-hospital care & coordinate discharge to home and other caresettings Provide,document, and coordinate patient and family education; serve as liaisonamong patients, families, physicians, and the care team Documentdischarge services accurately and timely in accordance with hospitalpolicy Maintainindividualized patient care plans and update them based on changes inpatient status Reviewdenials, identify appeal needs, and ensure appropriate appeal completion Reviewcontinued stay, medical necessity, medical reports and treatment plans Facilitateaccess to financial assistance, advance directives, legal aid, educationand medication assistance programs Coordinatepatient care and rehabilitation using consultation findings and socialwork expertise Partnerwith Quality Improvement to enhance patient safety, establish goals, andsubmit monthly quality reports Reviewpayer-requested charts and communicate clinical information to payers Maintainreferral source database & refer patients, families to communityresources Identifybarriers to patient progress & perform social work duties as needed Documentongoing physiological, psychosocial assessments, patient and familyeducation and responses & interdisciplinary care plan recommendations Validate,transcribe, and co-sign physician orders; report patient progress, andadvise physicians and departments on utilization management regulations Performutilization reviews in compliance with state regulations and evolvingutilization management regulations Monitordaily case management needs & maintain collaborative staffrelationships Communicatedischarge plans with physicians and complete follow-up calls within threedays of discharge Additional Responsibilities Assistwith coordinating quarterly utilization review committee activities andupdating policies and procedures Performregistered nurse duties in accordance with the RN job description Maintainconfidentiality and HIPAA compliance when handling sensitive information Adhereto organizational behavioral standards personnel policies and customerservice expectations Performother duties as assigned What We Are Looking For Qualifications Graduateof an accredited School of Nursing RNlicense in the State of Texas, Current CPR, BLS certification; ACLSpreferred 5+years of Registered Nurse experience; Case Management leadershipexperience preferred Knowledgeof utilization management, discharge planning, and regulatory requirements Additional Requirements Compassionateand patient-focused leadership Excellentcommunication, relationship-building, clinical judgement andproblem-solving skills Commitmentto quality, safety, and continuous improvement Abilityto multitask and perform in a fast-paced environment Abilityto stand, walk, bend, lift up-to 50 pounds, and assist patients as needed What You Can Expect PaidTime Off & Holidays Health& Supplemental Insurance DirectDeposit FitnessCenter Membership EducationReimbursement RetirementPlan If you're a collaborative nurse leader who thrivesin a fast-paced acute care environment looking to lead a dedicated CaseManagement team while making a lasting impact on patients and their families,we encourage you to apply today! #J-18808-Ljbffr The Staff Pad
