Description
  • Thoroughly assess patients in both inpatient and outpatient settings and create tailored treatment plans
  • Implement various physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
  • Collate efforts with nurses, physicians, case managers, and fellow rehabilitation specialists to secure optimal patient outcomes
  • Guide and inform patients and families about their therapeutic goals, discharge plans, and prescribed at-home exercises
  • Maintain accurate documentation of evaluations, treatments, and progress in alignment with facility standards and regulatory requirements
  • Engage in interdisciplinary rounds and conferences focused on delivering coordinated care
Education
  • Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
Certifications
  • Valid state licensure as a Physical Therapist
  • CPR certification preferred
  • Additional certifications in specialty areas (e.g., neurological, geriatric) are highly regarded but not mandatory
Skills
  • Prior experience in a similar role is strongly preferred
