Vivo HealthStaff is recruiting for a High Risk Clinician in Granada Hills, California. This is a permanent position with salary and benefits.

About the Opportunity

Our client, a rapidly growing healthcare provider in Southern California, is seeking a High Risk Clinician to provide comprehensive clinical care to high-risk patient populations in the San Fernando Valley and surrounding areas. This role leads post-discharge transitional care efforts, ensuring clinical stabilization, reducing preventable hospital utilization, and improving patient outcomes. Direct patient care is provided in a clinic setting with potential for home and community visits as needed.

Key Responsibilities

• Provide clinical care to complex, high-risk patients in a designated clinic, with periodic community visits when necessary.
• Lead post-discharge transitional care efforts, working closely with Primary Care Physicians and specialists.
• Participate in and lead interdisciplinary meetings to support patient care and ensure effective communication among healthcare providers.
• Oversee the clinical work of a High-Risk Nurse Practitioner conducting in-home visits.
• Refer patients to appropriate clinical, social, and behavioral health resources.
• Document all care provided in compliance with regulatory standards.
• Adhere to clinical guidelines, group protocols, and health plan requirements.
• Perform additional duties as assigned by management.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Board Certified/Board Eligible in Internal Medicine, Geriatrics, or Emergency Medicine (preferred but not required).
• Active California medical license and DEA privilege.
• Minimum of five years of clinical experience, with two years of managed care or health plan experience preferred.
• Proven excellence in both hospital-based and outpatient medical practices.
• Strong clinical knowledge, communication, and organizational skills.
• Ability to work in a fast-paced environment, independently and collaboratively.
• High personal ethics and the ability to build positive relationships with management, medical directors, and department heads.

Compensation and Benefits

• Salary: $270,000 – $300,000 annually, based on experience, skills, and location.
