Key Responsibilities Serve as the primary point of contact for intake and referral inquiries, ensuring timely, accurate documentation and follow-up. Build and maintain strong relationships with referral sources, patients, families, and internal clinical teams. Review clinical information gathered during initial inquiries to assist in level-of-care determinations. Schedule face-to-face level-of-care assessments within 24 hours of initial contact. Communicate Beacon’s services clearly, including program offerings, admission criteria, regulatory requirements, and insurance considerations. Facilitate referrals to community-based or alternative levels of care when admission is not clinically appropriate. Guide patients and caregivers through the admission process, including obtaining required consents and documentation. Obtain insurance pre-certifications and collaborate with billing teams regarding payer requirements. Track and maintain referral data, providing weekly reports to leadership. Collect and document patient and physician feedback, contributing to quality improvement initiatives. Participate in performance improvement activities as directed by leadership.
