Responsibilities
- Conduct comprehensive PT assessments for residents and develop individualized treatment plans aligned with short- and long-term goals.
- Provide skilled therapies to improve mobility, strength, balance, gait, and activities of daily living per plan.
- Deliver patient-centered interventions, adjust plans based on progress, and document sessions clearly in the chart.
- Collaborate with an interdisciplinary team, including nursing, OT, speech, and social work, to support safe, holistic care.
- Educate residents and families on exercises, equipment, safety strategies, and home exercise programs; document outcomes and progress.
- Ensure adherence to clinical guidelines, regulatory requirements, and facility infection control and safety protocols.

Requirements
- Active Physical Therapist license in West Virginia.
- Doctor of Physical Therapy (DPT) degree or equivalent clinical credential.
- Ability to practice in a Skilled Nursing Facility setting and complete travel assignment requirements.
- Prior experience in geriatric rehabilitation or SNF settings preferred.
- Strong communication, documentation, and collaboration skills; ability to work as part of a multidisciplinary team.
