Registered Nurse - Home Health

Job Description

Job Summary:
Provides in-home nursing care coordination and implementation for patients based on physician orders, ensuring quality care through the nursing process. Oversees and evaluates home health aides and LPNs/LVNs, supporting patient recovery and well-being.

Responsibilities:

• Coordinate and deliver home health nursing services according to physician prescriptions and agency protocols.
• Supervise and assess the performance of home health aides and licensed practical/vocational nurses.
• Document patient assessments, care plans, and progress accurately within agency systems.
• Travel to patient homes up to 75 miles from the clinic to provide care and follow-up visits.
• Collaborate with the healthcare team during office meetings and shift handoffs.
• Ensure timely completion of documentation within 24 hours of service.

Qualifications:

• Hold a valid and current RN license in the applicable state.
• Possess minimum 1 year of recent adult home health or hospice experience.
• Proficient in using HomeCare HomeBase (HCHB) EMR system within the last 6 months.
• Demonstrate proficiency with OASIS documentation for RNs.
• Be capable of traveling up to 75 miles and maintaining reliable transportation.
• CPR/BLS certification required.
