Licensed Practical Nurse (LPN) - Day Shift

Avondale Health and Rehabilitation Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our growing team in Humboldt, Tennessee . In this role, you will provide direct resident care, administer medications, and support our clinical team in delivering high-quality care in a skilled nursing and rehabilitation facility .

We are currently hiring for Full-Time and PRN Day Shift (6:30 AM - 6:30 PM).
Pay Rates

$22.00 - $28.00 per hour
$24.20 - $30.80 per hour MaxPay Program (ask during interview for details
LPN Requirements

• Active Licensed Practical Nurse (LPN) license in Tennessee
• Experience in long-term care, skilled nursing, or rehab preferred
• Strong clinical and assessment skills
• Excellent communication and teamwork abilities
• Ability to prioritize and work in a fast-paced environment
• Willingness to work weekends and holidays as needed

LPN Responsibilities

• Provide direct nursing care according to individualized care plans
• Administer medications and treatments as ordered
• Monitor resident condition including vital signs and changes in status
• Complete accurate and timely documentation
• Assist with admissions, transfers, and discharges
• Work closely with RNs, CNAs, physicians, and therapy staff
• Support residents with daily care needs when needed
• Participate in care planning and family meetings
• Follow infection control and safety procedures
• Help train and support new nursing staff
