Physical Therapist - Skilled Nursing Facility

Description Location: Bend, Oregon Schedule: Part-Time | 2-3 days/week $50-$58/hour + Supportive In-House Therapy Team At Bend Transitional Care , we’re dedicated to providing exceptional skilled nursing and rehabilitation services that empower our residents to recover, thrive, and live well. We’re hiring an Physical Therapist (Part-Time) to join a collaborative, therapist-driven team where your work directly impacts recovery outcomes every day. In this role, you will provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents/patients suffering from injuries or disease. Why Join Us Competitive Pay: $50-$58/hour (depending on experience) In-house therapy model and a part of our facility team Flexible scheduling with work-life balance in mind Life, short-term, and long-term disability insurance Continuing education support & career growth opportunities Leadership that values clinician input and patient-centered care What You’ll Do Establishes and administers a treatment plan with specific therapy goals determined according to the patient's capacity and tolerance under the direction of the physician Assesses patient needs; plans for, evaluates and modifies care to meet goals of physical therapy interventions Collaborates with all disciplines to plan and evaluate team goals for each patient Sets realistic and achievable goals for patients/residents Documents and records the patients’/residents’ condition and progress per state and federal regulations Educates patients and families in an appropriate therapy method Optimizes the functionality and skills of patients by providing, directing and/or overseeing the screening of all residents Communicates patient status and needs to the patient, family, and other involved professionals Ensures punctuality and regular attendance for assigned shifts What You Bring Active Oregon PT License (we support and reimburse licensing costs) Degree from an accredited Physical Therapy program Valid CPR certification SNF, post-acute, or long-term care experience preferred—but not required Strong clinical knowledge of therapy techniques and patient care A compassionate, team-oriented approach Work Environment & Expectations Collaborative, team-based therapy environment Productivity expectations aligned with industry standards, with leadership support Emphasis on quality care over burnout culture Ready to Make a Difference? Requirements Qualifications Education: Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program. Licenses/Certification: Current Physical Therapist license in the state employed. Current CPR certification.
