Aevum Home Health is a fast-growing and locally owned organization with an excellent reputation throughout San Luis Obispo county. We are rapidly growing and seeking a full time Certified Home Health Aid (HHA) to join our team and who bring both heart and skill to their work. The HHA will provide essential personal care and assistance with daily living activities to clients in their homes, supporting their health, safety, and independence. This role requires empathy, professionalism, and a strong commitment to patient dignity and quality of life.

Work schedule: Regular work schedule is Monday through Friday, 8am-5pm with a choice between 4 or 5 days/week, with a rotating weekend.

Duties & Responsibilities:

• Assist clients with personal hygiene tasks such as bathing, dressing, grooming, and toileting
• Support mobility and transfers, including walking and use of assistive devices
• Help with light housekeeping tasks such as laundry, meal preparation, and tidying living areas
• Monitor and report changes in clients physical or emotional condition to the supervising nurse or care coordinator
• Remind or assist clients with prescribed medications (in accordance with regulations)
• Provide companionship and emotional support
• Follow the care plan established by the nurse or case manager
• Maintain accurate documentation of services provided and client progress
• Adhere to infection control and safety protocols

Required Experience, Education, & Skills:

• Valid Home Health Aide Certification (CA) Required
• Reliable transportation and a valid drivers license
  
• Have and maintain Auto insurance coverage
• CPR certified (Healthcare Provider)
• Home Health &/or Hospice experience preferred
