Housing & Tenancy Navigator for IDD Support
OneWell Health Care
OneWell Health Care in Pennsylvania seeks a Housing & Tenancy Coordinator to help individuals with IDD identify, obtain, and maintain suitable community housing while following HTTSS guidelines. You will work with families, landlords, and Supports Coordinators to promote stability, independence, and person-centered service delivery, including lease education, move-in logistics, and ongoing tenancy support. This is a full-time, on-site role requiring travel within the community and diligent #J-18808-Ljbffr OneWell Health Care
