A well-established outpatient primary care practice in Augusta, Georgia is seeking a Primary Care Physician to join its care team. This opportunity is ideal for a physician who wants a stable outpatient setting, a consistent schedule, and a collaborative care model focused on quality outcomes, prevention, and long-term patient relationships.

This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes chronic disease management, preventive care, wellness visits, medication management, and continuity of care. Providers are supported by a team-based clinical structure designed to help physicians practice efficiently while spending meaningful time with patients.

✅ Provide comprehensive outpatient primary care to adult patients
\n ✅ Manage chronic conditions including diabetes, hypertension, COPD, CHF, and other common primary care needs
\n ✅ Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
\n ✅ Collaborate with APPs, nurses, care managers, behavioral health, and other clinical support staff
\n ✅ Support a care model focused on quality outcomes, access, prevention, and continuity of care
\n ✅ Maintain accurate and timely documentation in the EMR

✅ Board-certified or board-eligible Family Medicine or Internal Medicine Physician
\n ✅ Active Georgia medical license, or ability to obtain
\n ✅ Primary care, internal medicine, family medicine, or geriatric care experience preferred
\n ✅ Comfortable caring for adult and/or senior patient populations
\n ✅ Strong communication skills and a team-oriented approach to care
\n ✅ Commitment to preventive medicine, patient education, and long-term health outcomes
