Licensed Clinical Social Worker LCSW Care Manager needed to support hospice organization in Grand Rapids, Michigan and surrounding area. '..Key Responsibilities:

• Lead all program startup activities, including workflows, policies, and operational design
• Manage program budget, financial performance
• Supervise and support palliative care staff in collaboration with the Medical Director
• Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
• Support provider credentialing and payer contracting
• Ensure efficient scheduling, referrals, documentation, and care transitions
• Drive program growth, customer service, and performance improvement
• Deliver social work services during start up and as needed
• Participate in the Manager On-Call Rotation for hospice services..'
