Occupational Therapist: Home Health

Full Time Occupational Therapist (OT) Home Health Location: Discovery Bay and surrounding cities. All positions require field visits to patients’ homes; remote or work‑from‑home positions are not available unless specifically noted. Responsibilities In partnership with the Case Manager, you will provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively. Key areas of focus include home safety, medication management, wheelchair seating and positioning, pain and falls prevention, chronic disease and dementia management, and caregiver training. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.
