RN Case Manager - Discharge & Care Continuum Lead

Trinity Health Livingston in Michigan is seeking a Registered Nurse Case Manager to lead discharge planning and drive clinical, financial and utilization outcomes across the care continuum. This contingent, part-time role includes 0 FTE status with no benefits at the Howell/ Brighton campus. Orientation will run 6–8 weeks, 8 a.m. to 5 p.m., Monday through Friday.
