Join us in building a team that reflects the rich diversity of our society and let's make a positive impact together.Primary Work Location: Natalie's PlaceNatalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. Cardea Health’s transitional housing programs collaborate with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Natalie’s Place provides comprehensive supportive services, including onsite nursing and caregiving support, transportation to medical appointments, nutritious meals, housing navigation and placement assistance, and additional wraparound services designed to promote stability, recovery, and long-term housing success.Job Summary:The care coordinator will assist clinical staff and residents in the coordination of medical and behavioral health care.Job DutiesMaintain a calendar of upcoming resident medical and behavioral health appointments. Communicate upcoming appointments to clients, clinical staff, and transportation servicesAssist clinical staff and site-based residents in the scheduling of client medical and behavioral health appointments.Communicate with local pharmacies regarding site-based resident prescriptions. At the direction of nursing staff request medication refills, deliveries, and transfersRequest relevant medical records of site-based residents from hospitals and community clinics. Transmit medical records and other authorized requested documentation to partner clinics to facilitate care coordinationInterface with site-based resident healthcare coverage plans to avoid interruption in services and facilitate careTake telephone messages and provide feedback and answers to patient/physician/pharmacy callsEnsure adherence to infection control standards on site among staff and residentsAnswer calls to the nursing station and respond independently to when appropriateAssist nursing staff with light clerical duties including filing and making copies.Maintain confidentiality in accordance with the Health Insurance Portability and Accountability Act (HIPAA)Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respectOther duties as assignedConstant work around and/or with people OR Interaction with patients, clients, and external customersClinical setting in a permanent supportive housing siteMinimum Required Skills & Qualifications:High School Diploma or equivalentMust be authorized to live and work in the U.SMust be at least 18 years of ageMust have personal, reliable transportation to and from work sites and shiftsPreferred Skills & Qualifications:Prior experience with homeless or incarcerated populations is desirableSelf-directed and able to work with minimal supervisionDetailed oriented and highly organizedMust provide proof of COVID vaccination or willing to be vaccinatedPrior experience with homeless or incarcerated populations is highly desirableProficiency in Microsoft Outlook, Word and ExcelPhysical Requirements:Occasional sitting or stationary workFrequent standing, walking, kneeling, stooping, and ascending stairsOccasional lifting and carrying up to 20 lbsFrequent pushing and pullingFrequent reaching high, low, and levelWork Environment:In-person onsite at a supportive housing facility.
