In Home Healthcare LVN - Low Acuity - Full Time/Part Time Days

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, focusing on pediatric patients with needs such as feeding tubes and medication administration. Responsibilities Include Delivering and documenting nursing care according to physician orders and care plans. Monitoring and assessing patient conditions, reporting changes promptly. Implementing and updating care plans, taking appropriate actions to achieve outcomes. Educating patients and families on disease management and self-care strategies. Maintaining compliance with policies, regulations, and safety standards. Participating in ongoing education and ensuring infection control measures. Qualifications Include Current, active LVN license in the practicing state. CPR certification (with hands-on component). TB screening within the last 12 months. Reliable transportation and ability to perform physical tasks such as lifting and repositioning patients. Preferred: Six months of recent clinical experience, home health experience is a plus.
