Population Served Adult clients, age 18 and older, diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder, who may be experiencing housing instability or who are recently housed after experiencing homelessness. Specific Duties & Responsibilities Office- and field-based services with transportation of clients using agency or personal vehicles, as appropriate Work with clients with active mental health symptoms and within their treatment preferences Meet clients where they are in the community, including private homes or outdoors, promoting confidentiality in all settings Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals living in substandard conditions Assess and diagnose clients presenting for services to determine eligibility for CCBHC services and housing stability team Refer client to the most appropriate outpatient program according to assessment and diagnostic results, within or outside CCBHC Obtain consent for treatment from all responsible persons Establish appropriate and productive therapeutic alliances with the client(s) and caretakers, as applicable Complete safety assessments to include risk of violence towards self and others; complete safety plans and/or psychiatric advanced directives with clients Therapeutic services include assessments, comprehensive case plans and reviews, individual and group therapy, as appropriate Work collaboratively with community partners Embed outreach and other services in alternate locations and with community providers to reduce barriers to access Provide services in-person or via telehealth platforms Gather and complete all CCBHC data metrics for homeless population served CCBHC Model of Care Family & Children's Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by an innovative, team-based approach and whole-person care for adults and children with a range of complex mental health and substance use challenges. Utilizing Multi-disciplinary Teams, Staff Work Collaboratively To Provide a Coordinated Effort To Enhance Client Recovery.
