Location: Modesto, CA or Stockton, CA

Schedule: Monday-Friday, 8 AM-4 PM

Duration: 13+ Weeks

Duties and Responsibilities:

Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.

Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.

Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).

Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.

Order, track and manage maintenance for equipment.

Qualifications and Requirements:

Education/Training/Certifications:

Bachelor's Degree in Physical Therapy plus five years' of Progressively responsible PT experience. Master's Degree in Physical Therapy or Doctorate in Physical Therapy preferred. Valid California Physical Therapy License, or proof of License eligibility. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy. Valid State Driver's License with a clean DMV record. Copy of recent Vehicle Insurance required.

Skills and Experience:

Minimum of two years' Physical Therapy experience in a clinical setting with a frail or elderly population. Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs. Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations. Two years' supervisory experience with demonstrated ability to mentor and develop team members. Ability to process accurate information in a timely manner. Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.

Compensation: $70.00 - $80.00 per hour Minority and Women Owned Business Enterprise (MWBE) certified by the State of New Jersey and Minority Business Enterprise (MBE) certified from National Minority Supplier Development Council.
