Licensed Practical Nurse

Job TitleEssential Duties and ResponsibilitiesThe essential functions include, but are not limited to the following:Provide skilled nursing care under the direct supervision of the clinical supervisorComplete evaluation tasks including vital signs and medication reviewsCoordinate with physicians, therapists, and other individuals in the client's care planAssess and chart observations of the patient's condition at each visitMaintain complete and accurate documentation of nursing careWork collaboratively with all members of the health care teamProvide education to patients and families on proper home health care procedures and strategiesRecord patient's medical information and vital signsMonitor, record, and report symptoms or changes in patient's conditionsConsult and coordinate with health care team members to assess, plan, implement, or evaluate patient care plansAdminister local, inhalation, intravenous, or other anestheticsPrepare patients for and assist with examinations or treatmentsModify patient treatment plans as indicated by patient's responses and conditionsOrder, interpret, and evaluate diagnostic tests to identify and assess patient's conditionMonitor all aspects of patient care, including diet and physical activityAssess the needs of individuals, families, or communities, including the assessment of the patient's home or work environments, to identify potential health or safety problemsEngage in research activities related to nursingPerform other duties as assignedMinimum Qualifications (Knowledge, Skills, and Abilities)· Associates degree required; Bachelor's degree from school of nursing preferred· Current and valid Licensed Practical Nurse license· One year of LPN experience· Current CPR certification required· Valid driver's license required· Current automobile registration and insurance required· Strong interpersonal skills that build personal rapport and establish trust with the patient· Computer literate· Strong organizational skills· Excellent written and communication skills· Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)RequirementsValid LPN license, Prior outpatient or clinic nursing experience preferred & Strong clinical and communication skillsCompany InformationArbor Family Health is a Federally Qualified Health Center (FQHC) serving Pointe Coupee, West Baton Rouge and Iberville Parishes.Benefits Include:Competitive salaryHealth, vision, dental and life insurancePaid holidays and Paid Time OffContinuing education support
