Health/Human Service Case Manager II

In this dynamic role, the Health & Human Services Case Manager II will empower individuals and families by assessing needs, determining eligibility for critical public assistance programs, coordinating supportive services, and developing comprehensive service plans that promote stability, health, and long-term self-sufficiency. You will collaborate with community partners, employment and training providers, healthcare professionals, and multidisciplinary teams to connect clients with essential resources such as SNAP, Medicaid, TANF, childcare assistance, housing support, employment services, and behavioral health resources. The ideal candidate is skilled in case management, customer service, and problem-solving, with the ability to communicate effectively across diverse populations while maintaining accurate case documentation and ensuring timely, high-quality service delivery. This position plays a vital role in improving the well-being of vulnerable and low-income residents through advocacy, coordination, and compassionate support.
