California Licensed Telephonic RN Case Manager

Compassionate and dedicated, the full-time Remote Bilingual Spanish Telephonic RN Case Manager will provide telephonic case management for medically complex and chronically ill members, conducting health assessments and coordinating care from a remote work environment. Key responsibilities Provide telephonic case management to medically complex and chronically ill members Conduct comprehensive health assessments and create individualized care plans Coordinate care with internal and external partners, including physicians and specialists Required qualifications Active, unrestricted RN license in California (Non-Compact) Minimum 2 years of clinical nursing experience At least 1 year of case management experience Proficiency with Microsoft Office (Word, Excel, Outlook) Bilingual in Spanish preferred Virtual Vocations Inc
